PAYMENT OPTIONS:**
*Fees include services for up to four teeth collected and shipped simultaneously. If laboratory tests determine that samples are not viable upon receipt, then a credit will be applied to the account and a second attempt will be honored at no additional cost OR you can choose to receive a FULL refund.
